This is an individual assignment. With the program (hypertension) you will be evaluating in mind, each student should, individually, complete Worksheets A (Raw Material for Your Logic Model) and B (Beginning with the End in Mind) adapted from the CDC: Program Evaluation for Public Health Programs: A Self-Study Guidehere are a couple of overriding issues in your response to the assignment.
First, this program is to take place in the Sea Mar clinic in Seattle. the same interest holders are to appear in all three worksheets.
Worksheet A—Identify
A good list of interest holders. Given that this is a clinic-based program aimed at increasing adherence to hypertension medications, what role would you see for a community organization like the Center for Black Health & Equity?
Given that this program is pharmacist-centric, shouldn’t the program operations include pharmacists?
In the second part of Worksheet A and in Worksheet B, you list funding organizations. It would make sense to include some of these under users of the evaluation.
